The PATH Award

Each quarter NeoPath recognizes one of our exemplary clinical team members for making a difference each day by following our PATH.

Meet The PATH Award Winner: Paula Gottwalt

I serve as the Home Visit Nurse for the Prairie Island Indian Community, where I provide in-home nursing care, health education, and chronic disease support for Tribal members of all ages. Over the past year, I have:

-Helped initiate and set up the consultation and wellness space at the Prairie Island Elder Center and now provide scheduled and walk-in nursing services and special nurse mediated events, making care more accessible and convenient for elders.
-Partnered with Community Health Representative (CHR) to improve care coordination, medication reconciliation, and patient follow-up.
-Worked with the Dietitians to roll out a new diabetes pilot program (SDPI).
-Supported families through hospice transitions, post-hospital discharges, wound care, and complex medication setups to keep members safe at home.
-Begun planning future collaboration with Tinta Wita Tiwahe’s mother-baby program to better reach young families and make nursing care more convenient for parents.
-Built strong relationships with patients, families, and Tribal programs to create a culturally respectful, trust-based model of care.
